The only thing that can guarantee cleanliness is a person's faith & that's where Islam enters the equation. It doesn't matter if she's a superstar diva or your next door neighbor, if she ain't praying the 5 salahs, she ain't clean. Even here in the West, these women don't wash whenever they use the bathroom, they wipe and may wash when they're taking a shower. Instead they cleanse with a mixed solution of vinegar, water and additives once a week, while relying on odor neutralizing products in between. Because of the lack of washing after using the bathroom, many hospitals have taken up the policy of giving wash-bottles to post-partum female patients to wash everytime they use the bathroom and to take home with them. They're finally accepting what was revealed to us 1400+ years ago.
