Conversion to Islam Questions

Are there any additional requirements beside the person reciting the Shahada ? Does she have to get a certificate ? Or is it enough for one person to be a witness ? No need for a name change ?

Name changing is not required for a convert.

Witnesses are not required but it is recommended as it is a giant step in one's life.

Note: The Shahada should be given without a delay. Should Death come to them, they die as Muslims. Allah knows best.

So is it permissible to take photos or video of the event ? I am going to host the event after Eid at my crib.
