The skin is an ideal place for bacteria to flourish and cause infections which ofcourse if not treated would break/destroy that first barrier of innate immune system and lead to sepsis which is fatal.

i also think , he is mental ill. but who buys that cigarette for him , why not help him with food, shelter and cloths and above that to get help?!!

Even in the West although they're provided with money and homes... nobody cares about them.
You can see them on the streets drinking, taking drugs and going mad in the public spaces.
