It's a mind thing. Just look at it this way: when a drug user starts taking drugs it's a weak one and in small doses in the beginning and that's enough to give him/her high. As time goes by they have to increase the dose as well as the type of drugs they use (from weak-ass stuff like weed & alcohol into heroine and meth etc) coz the weak stuff wont give them that high they want as their system adopts it.

Sex is similar in the sense that in first the conventional methods are hot enough but as time goes by and porn "actor" has sex with multiple female "actresses" the soft-core and other conventional styles wont do for them and they have to do hardcore stuff. And even the hardcore shit wont be good enough and then that's when they go into some weird, fetish shit and once they reach there all that's left for them is impotence.
